Let’s Talk About Sex- How often, what’s normal & how to boost that libido

Sex is something that nearly every human on this planet gets to experience, and for each person, the experience of sex can be different. Many people have different perceptions on this important topic, so lets talk about sex, how often, what’s normal and how to boost that libido in those having issues.

As a healthcare practitioner who is a Master in Women’s Health Medicine and a Master of Reproductive medicine, I have to talk about the topic of sex on a daily basis. A lot of people come into my clinic (both men and women) with varying expectations and thoughts on what is normal, when concerning sex. Unfortunately a lot of people are way off the mark when it comes to a healthy sex life and what a healthy libido should be.

Libido is influenced by our health

If you do not have a healthy libido, there could be something wrong with your health. Yes, what you eat, think, do etc, all plays a part in your sexual appetite. But, we can all have differing libido’s and it is important to understand this and seek help if you are having issues. A healthy libido is a sign of a health body and healthy mind.

What defines a healthy sex life

The average healthy human being in a healthy relationship should be having sex at least 1-3 times per week, depending on age etc. I know a lot of you are lucky to be having sex 3 times per month.

What’s concerning about this is, some of these people are trying to have a baby and can’t understand why they aren’t falling pregnant. The answer is obvious, but, I’ll say it anyway, you have to have sex regularly to fall pregnant and it has to be at the right time. You need to be trying every day, or every second day of your cycle as we now know ovulation just doesn’t take place mid-cycle only.

Clinical research shows that more than 70% of people are ovulating before day 10 and after day 17 of their cycle. So if you are just trying it that small window of day 10-16, this could be the reason you aren’t falling. Btw, don’t stop having sex just because you are doing IVF. Sex (climax in particular) helps with implantation so stopping having sex is only hindering your chances of conception. I have talked about this is a previous post (click here to read)

We also now know that sperm also play a part in a health menstrual cycle as they help to trigger certain hormones to trigger ovulation and thus help with regulation of the menstrual cycle. It may explain why women using barrier methods of contraception have more issues with irregular cycles and menstrual issues. Not that we are promoting unsafe sex mind you. Why is climax/orgasm so important

Climax also stimulates blood flow into the uterus; helps trigger certain hormones and also helps with implantation. Climax also helps ones libido and it can even help in the treatment of depression.

We also hear the jokes about men and their ever-constant desire to have sex, but most of the time this is greatly exaggerated. In practice I am actually seeing that women are now the ones with the higher libidos and it is the men that are having all the problems. Stress is a big factor these days. We are also seeing many men with testosterone deficiency. Being overweight and unhealthy physically and mentally can be a factor also.

It has also been shown that regular sex, even if scheduled, actually helps with the libido and helps with the desire around wanting more sex.

Understanding differing libido’s

Men and women are different when it comes to libido and the desire around sex. Men need to understand that it can often take all day for a woman to arouse her desire around wanting sex. Men need to know that women need to be wooed, given playful thoughts, mind play and a place to feel safe to get her into the mood for sex. Guys, apparently helping out around the house also helps (hint, hint)

Intimacy is more than just sex

Remember sex is a healthy part of a relationship and a healthy libido shows good health. Sex is also a way of connecting with your partner and couples should find way to prioritise each other so that this connection happens regularly. It is also about intimacy and that sacred intimacy, which is on a whole other level to the physical act of just having sexual intercourse.

What affects a libido?

Many people do talk about having differing libido’s, or having trouble with libido, and it is important to understand what may be affecting ones sex drive.

So, what are some of the main things that effect libido?

  • The Pill – First and foremost, the pill for women. Anything that decreases fertility can decrease your sexual desire. The pill also turns off certain receptors responsible for conception, which can then turn off the libido.
  • Stress – Constant low grade, or high grade stress, is a big problem when it comes to loss of libido. It can cause impotence & erectile dysfunction in men and loss of libido and menopausal symptoms in women.
  • Medications – Drugs such as anti-depressants, the pill, anti-inflammatories, and lots of other medications can ruin your sex life. They can also delay or stop orgasm. There are some that can improve it too. Not many!
  • Alcohol and recreational drugs– This one is pretty self explanatory. We’ve all heard the saying ‘Fosters flop’ from drinking too much beer. Drugs such as marijuana, speed, and other illicit drugs can all impair libido
  • Sexual Abuse– A history of sexual abuse can definitely affect the sex life. This needs to be dealt with by speaking to a counsellor, psychologist or sex therapist.
  • Health Problems– Health problems such as Low Thyroid, Diabetes, being overweight, depression, eating disorders, can interfere with your libido and sex life.
  • STD’s– Some STD’s are often undetected and cause pain and discomfort during sex, which leads to not wanting to have sex. Many of these can be cleared up with medication so that your sex life can return to normal. Some STD’s are there for life but can be controlled. STD’s such as herpes and HIV don’t have to stop your sex life.
  • Gynaecological issues – Some gynaecological disease states such as Endometriosis and Adenomyosis can make sex painful and cause a lack of desire. Have a read of my article about painful sex. Women with PCOS can have low libido as a result of hormonal fluctuations and insulin resistance.
  • Diet– This is a big one. If you eat the wrong foods the body will respond accordingly. Your sex life can be ruined by what you put in your mouth to sustain your body.
  • Exercise– Over-exercising can stop you getting a period and cause fertility issues in both men and women. Reduce the exercise and the libido will respond accordingly. Exercise can also increase libido. It is about moderation.

Final Word

If you are having trouble with your libido there are many medical and natural products that have been shown to assist with low libido and help with a healthy sex life for both men and women. It is important that you talk to a qualified healthcare practitioner about issues to do with sex and libido. You need to see an expert who is trained to know issues that may be causing your particular issues. The can also give you dietary and lifestyle advice to help in this area too.

Sex Around The Time of Embryo Transfer Increases The Likelihood of Successful Early Embryo Implantation and Development.

Research has now shown that sex around the time of embryo transfer increases the likelihood of successful early embryo implantation and development.

Intercourse during an IVF cycle has the potential to improve pregnancy rates and there is adequate research to now back this up. We know that in animal studies, exposure to semen is reported to promote embryo development and implantation.

Intercourse may assist implantation

This is actually good news for humans as well as it shows that intercourse may act to assist implantation. Animal studies reveal that exposure to seminal plasma, the fluid component of the ejaculate, is particularly important for achieving normal embryo development and implantation. Animals that become pregnant through artificial insemination or embryo transfer without being exposed to seminal plasma have substantially lower rates of implantation than those exposed to seminal plasma (Pang et al., 1979; Queen et al., 1981; O et al., 1988; Flowers and Esbenshade, 1993), while rodents inseminated with spermatozoa prior to blastocyst transfer also have a higher rate of implantation compared with those not exposed to spermatozoa (Carp et al., 1984).

Intercourse may influence pregnancy success rates

A multicentre prospective randomised controlled trial was conducted through IVF centred around the world, including Australia. The study was conducted to determine if intercourse around the time of embryo transfer, or just before and embryo transfer in an IVF cycle, actually has the potential to have any influence on pregnancy success rates.

Participants in Australian IVF clinics underwent frozen embryo transfer (FET) and participants in Spain IVF clinics did fresh embryo transfer. Participants were randomised to either have intercourse, or to abstain from intercourse around the time of embryo transfer.

The study showed that there was no significant difference in the pregnancy rates between those couples that abstained and those that had intercourse. However, the portion of transferred embryos that made it to 6-8 weeks gestation was significantly higher in the women exposed to semen compared to those who abstained.

This landmark multi-centre international study showed that women who had sex around the time of embryo transfer, and who were exposed to semen around the time of embryo transfer, had increased likelihood of successful early embryo implantation and development.

Couples need to be having more sex during IVF cycles

One of the things that I always promote as part of my fertility program, is that regular sex is so important for our fertility patients, on many levels. Sometimes the obvious eludes some people though.

One of the things we see quite regularly is that couples doing Assisted Reproduction (ART) are abstaining from sex fearing it will affect their chances of conceiving. Actually the opposite is true. By not having sex during ART cycles (IUI, IVF etc) you are affecting your chances of conception.

I have spoken about the importance of sex and orgasm assisting implantation in other posts and there is so much medical research to back this up. This is seen in the research I have talked about above.

Let’s be real and look at the facts

Let ask the question “If you were trying naturally, would you stop having sex for fear that conception has taken place?”

Then why would you stop having sex around an ART cycle?

Let’s face the facts, implantation takes place in the uterus, and not the vagina, and no man is that well endowed to even penetrate the cervix, so…. Let’s get a grip here

The question to ask is “What does an embryo feed off and need to successfully implant?”

The answer is blood!

Think of a tick borrowing into skin to feed off its host.

How do you get blood flow into the uterine lining?

The answer is that sex and climax stimulate blood flow to the lining to assist implantation and also prepare the lining for implantation. Nature has given us all the tools for healthy conception to take place, and yet many of us just aren’t using them.

Sex is more than just and egg and a sperm

Don’t forget that regular sex during this time not only helps assist implantation, but it also helps with the bonding process and physical connection process during this stressful time. Many couples split up because of losing this connection during the ART process and sex is a way of keeping that physical and emotional connection. Sex also tells your body you are also preparing to conceive on another level too.

For those doing IVF etc, next time you are doing an ART cycle maybe it is time to start doing things the way nature intended to give you that boost you so desperately are needing. Go get busy people

Let’s Talk About The Health Of The Vagina, The Pelvic Floor & Stone Eggs

17 Reasons You Should Having Regular Sex and Orgasms

Part of my job as a healthcare practitioner with a special interest in and reproductive conditions, and women’s and men’s health conditions, is to talk about sex, talk about climax and the benefits of this for a couple, or an individual.

Regular sex, or regular climax, can help have many benefits for an individual, with it helping everything from menstrual pain, right through to increasing pregnancy rates, regulation of the menstrual cycle and even helping moods, sleep and helping your live longer

Here are 17 reasons why you should be having regular climax, or regular sex and many of the health benefits that go with it.

1.It’s Exercise
It’s not necessarily a full workout, but it can be as good for you as moderate exercise. It raises your heart rate about the same as a brisk walk or a slow bike ride.

2.Good for a Woman’s Heart
Women who have sex a couple of times a week are less likely to get heart disease than those who have it once a month. Whether that’s because healthier women enjoy it more often, or because it helps protect a woman’s heart is unclear.

3.May Help Your Headache
Say goodbye to the old standby “Not tonight, Dear. I have a headache.” It turns out sex can help with pain, and that includes some kinds of headaches, such as migraines. Not feeling frisky? Try: “Not tonight, Honey. I have a highly contagious stomach bug.” Works every time.

4.Lowers Stress
People who have more sex are less anxious when they’re faced with stressful tasks like public speaking or arithmetic. But according to the study, it only works when you have a partner — masturbation doesn’t count.

5.You May Live Longer
One study suggested that married women who climaxed more often had a slight tendency to live longer. Researchers aren’t sure if the sex actually lengthens your life or it’s just a side effect of a healthy lifestyle. But why take any chances?

6.Sharpens Your Mind
Sex has been linked to the making of new brain cells, and that’s a good thing. People over 50 who had more sex were better able to recall numbers and do basic math, and the difference was pretty big. It seemed to help men more than women, but both did better than those who had less sex.

You don’t have to overdo it — once a week is plenty. More than that, and the effect fades. But scientists only studied couples in committed relationships, so if you’re trying to meet your quota by picking up strangers at your local bar, all bets are off.

The hormone oxytocin is released during sex, and it sparks feelings of intimacy, affection, and closeness with your partner. That helps build a strong, stable relationship, which is good for everyone.

9.Keeps You Lean
The more sex you have, the slimmer you’re likely to be. Is that because more sex keeps you trim? Or because lean people have more sex? Scientists don’t really know, but all you need is a partner and a bathroom scale to try to find out.

10.Good for Mental Health
Adults in committed relationships who have more sex are less likely to be depressed or take medication for mental health issues. Orgasm also helps with the release of beneficial hormones such as oxytocin and these help us feel euphoric and also help with other endorphins in the body. These then make us feel better emotionally.

11.May Help Your Immune System
Move over, vitamin C. College students who had sex twice a week had more cold-fighting antibodies in their saliva than those who had sex less often. Couples who have regular sex have healthier immune systems and tend to be healthier overall.

12.Helps You Sleep
Orgasm triggers a surge of endorphins and oxytocin in both men and women, and that dulls pain and relaxes you. Both of those can help you sleep more easily, though according to scientists — and many women — the effect is more pronounced in men.

13.Lowers Risk of Cancer
Men who have more sex may be less likely to get prostate cancer, and women less likely to get breast cancer. Pregnancy and contact with sperm are both linked to the lower risk in women.

14.You Could Make a Baby
If you’re trying to have a baby, the more sex you have, the more likely you are to hit the right time of the month. But more sex may also prime women for pregnancy and improve sperm quality in men, which can speed things along. An egg has but 24 hours to be fertilized otherwise it dies. We also know that ovulation does not occur just mid-cycle either. Couples doing IVF also need to have regular sex as climax helps with implantation (by hormones and increased blood supply into the lining) and sperm also assist in helping with implantation as well. Many couples stop having sex during IVF and this is where they are going wrong.

15.It helps regulate your menstrual cycle
Regular climax nourishes the uterine and vagina with blood flow, but it also stimulates hormones, which help to also nourish the uterus, vagina and reproductive organs. Oxytocin helps with moods but it also helps with keeping your cycle regular. Research has also shown that sperm actually help to trigger ovulation.

16.Climax helps with Gynaecological issues and menstrual pain
Regular climax (not just sex with penetration) helps with increasing blood flow into the pelvic cavity, the uterine lining, vagina and surrounding areas which helps to keep your reproductive organs and muscles healthy. Hormones and increased blood flow also help with pain and circulation too. Climax helps with the microcirculation of the reproductive organs and also helps with keeping ovarian function regular and healthy too. As said before regular climax can help trigger ovulation, keep the cycle regular, but sperm can also help with this also. We do know that some women with endometriosis and adenomyosis do have pain with penetrative sex, but climax does not necessarily need for penetration to happen.

NB-  If you do have pain with intercourse, please make sure you get investigated properly as there are treatments and management to help this.

17.Helps Your Future Health and is Anti-Aging.
People who have more sex may have better quality of life — and not just now, but in the future, too. If you have an active sex life in middle age, you’re more likely to keep it up as you get older, which is linked to better health and happiness. Regular sex and climax is also anti-aging.

Final Word

Please remember that sex and regular climax has more benefits to it that the actual enjoyment part of it. It helps with intimacy, helps bring couples closer together and it helps with many other health benefits in the body. In practice I am seeing a huge disconnect with both men and women and their reproductive organs and this is why we are seeing so many issues with gynaecological conditions, couples not being able to conceive, relationships issues and issues with peoples health and lifestyles.

In a healthy relationship, couples should be having sex at least 1-3 times per week and if you are trying to have a baby, you should be having sex everyday and be going for it multiple times per day to increase your chances of conception. Maybe it’s time to turn off the TV, close the computer, turn off the mobile phone and spend more time in the bedroom with your own lives, then watching someone else’s. You will be healthier and happier for it in the long run.

Early Intervention & Early Management Is Vital For Gynaecological Conditions & Menstrual Issues

By now many of you would know my stance on Period Pain not being normal and that the sooner you get the cause treated and managed the better one is going to be in their day to day life.

Unfortunately not everyone knows that Period Pain is not normal and neither are some of the other symptoms women get each month with the onset of their menstrual cycle. Having heavy bleeding, bleeding in between cycles, menstrual cramps, severe pain, irritable bowel like symptoms, dark clotting, ovulation pain, bowel and bladder pain and urgency etc, are all not normal symptoms that a woman should endure with her cycle. Getting these symptoms at any time of your cycle is not normal either.

Early intervention and early management is the key to any disease state in the body and this definitely applies to menstrual issues and gynaecological disorders. Once a disease is expressed into the body, it can be very hard to treat, especially if it is left a long time and then inflammation spreads to other parts of the body, or in close proximity to where the initial disease was first expressed.

One of the reasons that prompted me to do this post was after a young woman, now in her 30’s, had contacted me and thanked me for helping her back when she was in high school. Since then I have known all of her family well and helped with maintaining their health. At the time she was about 14 years old and showed all the signs and symptoms of endometriosis. She was in so much pain each month, when her cycle came, and she was often curled up on the floor with nausea and vomiting from the pain. Everyone, including GP’s etc, had told her this is normal and that she needed to get used to it. They also told her that she is too young to have endometriosis, or any major gynaecological condition.

That is so bad. Please, please know that period pain IS NOT normal and that teenagers are not too young to have endometriosis. To be honest, they are now finding endometriosis in young girls under 10 years old. Many gynaecological issues can start very early on in a woman’s life, especially if there are hereditary factors involved.  Gynaecological and menstrual issues can be passed from generation to generation, so if mum, or your grandmother, or someone in your family tree had menstrual issues, or a gynaecological condition, there is a good chance that you may inherit this as well.

The long and short of it all was that her mother was also getting frustrated at everyone not helping and somehow ended up finding out about me and ended up in my clinic. From there I got her into one of the advanced trained laparoscopic surgeons I work closely with as soon as possible and this is where stage 4 endometriosis was found and excised properly.  Without coming to see me, this poor girl would never have found the cause of her menstrual pain and associated symptoms. I then did all her management of her disease moving forward. The main thing that this young girl and her mother were worried about was how this was affecting her education and daily life, but how this could also affect her future fertility.

The one thing I know is that the sooner there is intervention and treatment, the better the prognosis for a woman’s future fertility is. The one thing I do know is that endometriosis doesn’t always cause infertility, but it can make it harder to fall pregnant, if it isn’t managed early enough. The longer you leave a disease in the body untreated, the worse it gets, and then the symptoms get worse and the worse the future outcomes may be.

Lucky for this young lady is that she did have early intervention and management and she has proudly messaged me to tell me that she has had her 3rd child and that she puts it all down to me helping her when she was younger. I have many women message me and tell me much the same thing. It is so important not to leave these things just because you are being told it is normal. What the hell is normal about being in so much pain that you feel like you could die?

All too often I see women having gynaecological conditions, like endometriosis, missed and dismissed and that the longer that the disease has been dismissed, the harder it is going to be to treat. That is a sad fact for many women and some will have to endure repeated surgeries due to being missed and dismissed and have a life of issues, if their issue isn’t managed properly either. Many have not seen the right healthcare professionals, or the right surgeon either.

I have discussed in many of my previous posts.  Please have a read of my previous posts about this subject and the seeing the right team of people. This is why it is so important to have early intervention and also see a proper advanced trained laparoscopic surgeon who specialises in the excision of disease states like endometriosis. Then there needs to be proper management and treatments and lifestyle changes administered to help with suppression of the disease state, helping with inflammation and improving quality of day to day life.

There needs to be a multimodality/team like approach to the management of women with gynaecological issues, as there is no one single fixes all approach, with any medicine. While surgery may be a necessary part of the overall management of disease states such as endometriosis, it isn’t the saviour that many perceive it to be. Surgery does not sure endometriosis and there is no cure for the disease at present time. Once you have it, it is there for good.

Surgery is a necessary but small part of the overall picture that needs to combine many other treatments and modalities to give the best outcome for a woman overall. Once the surgery is done you need to look at managing and suppressing the disease and this is done by lifestyle changes, dietary changes, acupuncture, physio, herbal medicines, hormone therapies, pilates, yoga, pelvic floor and core exercises and many other modalities depending on one’s individual symptoms.

When I treat women with gynaecological conditions, or menstrual issues, I make sure they all get an individualised, person centred, caring approach tailored to how they are presenting rather than a one treatment for all approach that many seem to get. You won’t get the results you need that way because we are all individuals with different needs and different symptoms overall.

Lastly, please remember that period pain and menstrual irregularities are not normal and that the earlier you get onto it and get it treated and managed, the better your future outcomes will be.

Stop Telling Women That Period Pain is Normal

After seeing my 10th case of misdiagnosed Endometriosis this week, and goodness knows what else, I can say that I am well and truly over it and about to scream.

I am about to scream if I hear that one more woman, young or old, is told by their healthcare professional, GP, specialist, best friend, mother, facebook buddy etc, that period pain is normal.

Period pain IS NOT normal. It is far from normal and we all need to stop telling our daughters, sisters and women of this world that it is.

I think that anyone that says that need to come and work with me for a day and see the ramifications of women believing that period pain is normal, just because their doctor, healthcare professional, friend, or mother told them that it is.

I think I should post up some rather gruesome pics of women’s reproductive organs stuck together, their insides bleeding, and their pelvis completely obliterated. Yes, obliterated. That was the words that one of the surgeons used today to explain the insides of a woman that had been told that there is nothing wrong, just suck it up, scans haven’t found anything and just go on the pill and btw, period pain is normal.

No it bloody well isn’t (sorry for swearing but time for diplomacy is over). Women need a voice and need to be heard. Some of these poor women may not be able to have children, or have a healthy sex life, or be able to feel the pleasure of wonderful sex without pain, or ever hold their own baby, because they have been told to suck it up and be told that period pain is normal.

Period pain IS NOT normal and the sooner we get everyone to know this important fact the better. Sure, a little bit of discomfort can be normal. By that I mean just a tiny bit of pressure and basically knowing your period is about to come. But pain…. That is not normal. If you, your friend, your daughter, your sister, your wife, or any other woman you know, has to have days off work, days of school, is laying on the floor in pain, taking pain killers to get through the day, or beginning of their period, then that IS NOT normal.

Please get them to get a referral and see a good specialist who will listen to them and not dismiss them and may miss a gynaecological issue that could affect them for the rest of their lives. No… scans and blood test etc, do not always find the cause of period pain. Have a read on my other posts about this.

Seminal Fluid Improves Fertility in Women and Improves IVF Pregnancy Rates

Seminal fluid is often viewed as simply a vehicle to carry sperm to fertilize the female egg, but a more complex function in influencing female reproductive physiology is now evident.

Remarkably, seminal fluid contains special signaling agents that interact with the female reproductive tract to prime the immune response, with consequences for fertility and pregnancy outcome. This research isn’t new and we have spoken about this in previous posts.

But we now know a bit more about how sperm may assist in implantation for, especially for IVF.

Recent studies demonstrate a key role for seminal fluid in enabling  embryo implantation and optimal placental development. In particular, seminal fluid promotes health immune responses which facilitate embryo implantation by suppressing inflammation, assisting uterine circulation and blood supply and also protecting the embryo.

There is emerging evidence  where seminal fluid provokes an adaptive immune response in the cervical tissues after contact at intercourse, and spermatozoa accessing the higher tract potentially affect the endometrium directly. Research has now shown that sperm and seminal fluid actually help with endometrial receptivity, which is needed to create the right environment for a healthy embryo to implant.

These biological responses may have clinical significance, explaining why

[1] intercourse in IVF ET cycles improves the likelihood of pregnancy

[2] inflammatory disorders of gestation are more common in women who conceive after limited exposure to seminal fluid of the prospective father

[3] preeclampsia incidence is elevated after use of donor oocytes or donor sperm where prior contact with conceptusalloantigens has not occurred.

Research like this is important to define the mechanisms through which seminal fluid interacts with female reproductive tissues, to provide knowledge that may assist in preconception planning and infertility treatment.

It also bring attention to the fact that couples still need to be engaging in regular intercourse during IVF cycles to help not only with implantation, but to increase pregnancy rates through optimisation on the endometrial lining through contact with seminal fluid.

Regular intercourse also promotes connection and bonding for the couple as well and this is something that is very much overlooked.

Pain With Sex Dyspareunia

Do you suffer pain with sex? (dyspareunia)

Pain with sex, Intercourse pain, or dyspareunia, can cause problems in a woman’s life and it can be a cause of problems in couple’s sexual relationship. In addition to the physically painful sex, there is also the possibility of negative emotional effects. Then even when a woman may feel aroused and wanting sex, the fear of the pain can cause the whole process of wanting sex to stop.

How many people get Pain with Sex

It is estimated that about 20%-25% of women suffer vaginal pain with foreplay or intercourse. Pain can be acute, intermittent or chronic and can stem from a wide variety of causes that will be covered shortly. Unfortunately when women complaining of pain during sex, they are often dismissed as being inhibited, having psychiatric issues, or merely just making it up to get out of having sex. Many men would like to boast that it is them being well endowed that is causing the problem (they wish) but in fact if your partner is in pain, then you need to actually stop and listen to the reasons why. The fact is most of the time, their symptoms are related to legitimate medical issues that need to be investigated and treated accordingly. So guys, get your hand of it and start listening to your partner if she says she is getting pain.

What causes pain with Sex (Dyspareunia)?

In many cases, a woman can experience painful sex if there is not sufficient vaginal lubrication. There could be many reasons for this and one that is commonly seen in menopause. When this occurs, the pain can be resolved if the female becomes more relaxed, if the amount of foreplay is increased, or if the couple uses a sexual lubricant. Issues like this can easily be overcome, but there are some medical and gynaecological issues that could be causing the pain and being very much overlooked.

So what are the other causes of Pain with Sex?

Endometriosis – This is a condition in which the endometrial like tissue (lesions) that lines the uterus grows outside the uterus. It can cause all sort of pain in the pelvis, bowel and rest of the body, but it can cause pain with sex. It is one of the leading conditions that does cause pain with sex. Many women with endometriosis may not have symptoms of it, or may only have one symptoms like pain with sex. Symptoms do not correlate to the extent of the disease either. Some people with small amounts get lots of pain, while others can have lots of it and have no pain. Endometriosis can only be properly diagnosed via surgery (laparoscopy)

Adenomyosis – is a condition which is very similar to endometriosis. It is a conditions in which the inner lining of the uterus (the endometrium) breaks through the muscle wall of the uterus (the myometrium). Adenomyosis can cause menstrual cramps, lower abdominal pressure etc, before the menses and can result in heavy periods.  It can also cause pain with sex. The condition can be located throughout the entire uterus, or localized in one spot

Vaginismus-  This is a common condition. It involves an involuntary spasm in the vaginal muscles, which closes up the vagina and prevents penetration from happens. It is sometimes caused by fear of being hurt.

Pelvic Floor HypertonusPelvic floor hypertonus occurs when the muscles in the pelvic floor become too tight and are unable to relax. Many women with an overly tight and non-relaxing pelvic floor experience pelvic health issues such as constipation, painful sex, urinary urgency, bladder issues and pelvic pain.

Vaginal Infections-  These conditions are common and include yeast infections such as thrush and candida and these can cause inflammation to the vagina and cause pain with sex and also localized bleeding.

Vaginal skin conditions– Dermatitis around the vulva and also a condition called Lichen Sclerosis can all cause pain with sex due to the inflammation of the skin.

Atrophic VaginitisAtrophic vaginitis is a vaginal disorder that usually happens after menopause, but it can happen long before this time as well. When estrogen levels fall, the vaginal walls can become thin, dry, and inflamed. This then causes the surrounding tissue to lose its elasticity and become atrophied.

This can be uncomfortable and it can make sex uncomfortable, or extremely painful

Problems with the cervix (opening to the uterus) –In this case, the penis can reach the cervix at maximum penetration. So problems with the cervix (such as infections) can cause pain during deep penetration.

Problems with the uterus – These problems may include polyps, cysts, fibroids etc that can cause deep intercourse pain.

Problems with the ovaries – Problems might include cysts on the ovaries, or tubal disease.

Pelvic Inflammatory Disease (PID) – Often referred to as penis injected disease. With PID, the tissues deep inside become badly inflamed and the pressure of intercourse causes deep pain.

Ectopic Pregnancy – This is a pregnancy in which a fertilized egg develops outside the uterus, or into the tubes. It can cause immense pain and even death if not death with properly.

Menopause- With menopause, the vaginal lining can lose its normal moisture and tone and become dry. The vagina, uterus and surrounding organs can all suffer atrophy, which can cause bleeding and pain (see atrophic vaginitis above). It can also cause prolapse.

Intercourse too soon after childbirth, or surgery – Trying to have sex too soon after childbirth, or a surgery,  can cause pain during sex.

Sexually Transmitted Infections (STI’s) – These may include chlamydia, genital warts (HPV), genital herpes, or other STI’s.

Injury to the vulva or vagina- These injuries may include a tear from childbirth or from a cut (episiotomy) made in the area of skin between the vagina and anus during labor.

How Can Painful Sex In Women Be Treated?

Some treatments for painful sex in women do not require medical treatment. For example, painful sex after pregnancy can be addressed by waiting at least six weeks after childbirth before having intercourse, or when a women feels she is ready again. Make sure to practice gentleness and patience.

In cases in which there is vaginal dryness,  or a lack of lubrication,water-based lubricants will help. In the cases of some conditions such as menopause, topical estrogen creams may be needed to bring tone back into the vaginal wall, along with lubricants during sex.

Some of the conditions such as endometriosis, PID, fibroids, or trauma to the vagina and will require surgery and adjunct therapies such as hormones and other medications.

Other issues such as vaginal infections, bacterial infections, skin conditions, STI’s etc may just need medications such as antibiotics, antifungals, steroids and other medications for their treatment

Some conditions like vaginismus, or psychological traumas (sexual abuse), may require a person to see a counselor, psychologist, psychiatrist, or sex therapist.

There are also natural medicines such as Acupuncture, Chinese Herbal medicine,  herbal supplements etc which may assist with pain, either on their own, or in combination with medical treatments and talk therapy

Final Word

If you do have pain with sex, you need to go and speak to your healthcare provider, or seek the help of a gynaecologist, or women’s health expert.

Pain with sex is not normal, and you need to find out the cause of these issues and not put it off. Many times there are non-invasive and easy treatments to help.

Facts About Sperm Health and Their Lifespan

There is a lot of misinformation with regards to facts about sperm health and their lifespan.

Typically, millions of sperm cells are produced in the testicles every day. During this time, many things can affect their formation and interfere with their quality and maturity.

It takes around 90-120 days for a sperm to become motile

From a tiny sperm cell it can take between 90-120 days before a sperm is fully mature. The sperm eventually develops a head and tail, so that its cells start to resemble the shape of a tadpole.

The head contains all of the DNA, or genetic material, and the sperm uses the tail to help it move. A sperm doesn’t reach full motility until it actually reaches the egg, where when touching the egg it creates a reactions that causes “super motility” to give it the final power to push inside the egg. This is called the acrosome reaction.

There has been lots of debate about the actual time it takes for a sperm to mature and become fully motile, but general consensus is that it will take somewhere between 90-120 days.

This is why it is important that men look after their health long prior to conception, because the sperm they ejaculate today was created around 90-120 days ago and what they did to their bodies at that time, will influence how healthy those sperm are.

A man’s lifestyle affects his sperm quality

So if a man had a poor diet, was drinking, smoking, had heaps of stress and goodness knows what, this can all have an impact of the maturing sperm and this can result in poor sperm quality and damage to the DNA of the sperm, which will then be carried onto his offspring, should the sperm be successful in fertilising an egg. This is why the couple need to be healthy prior to conception, not just the female.

Biology 101 tells us it take a sperm and an egg to make a baby, not just an egg. Sperm quality issues make up a big part of fertility issues and they can also be a big part of miscarriage issues too. I have spoken about this many times in previous posts. Up to 85% of miscarriage issues can be related to chromosomal and DNA factors related to poor quality sperm and this is often very much overlooked.

What factors impact on sperm health?

There are always many factors that can affect the sperm formation process and interfere with sperm quality and the DNA of the sperm.

Health and lifestyle factors

  • Recreations drugs, medications or alcohol use
  • job, or occupation
  • tobacco use, or smoking in general (including recreational drugs)
  • stress
  • overheating the testicles (spas, saunas, bike riding)
  • excess weight gain and excess body fat
  • Trauma
  • Bike riding (due to heat and trauma through the seat of the bike)
  • Poor diet and nutrition
  • Excess sugars and additives
  • Preservatives and artificial colours and artificial sweeteners

Environmental causes

  • exposure to industrial chemicals
  • heavy metals
  • radiation or X-rays

Medical reasons

  • infection of the testicles
  • cancer of the testicles
  • swelling of the veins (varicoceles etc) that drain blood from the testicle
  • hormone imbalances
  • physical problems in the tubes that carry sperm through the reproductive system
  • chromosomal or genetic disorders (such as Kleinfelters syndrome)
  • certain medications
  • surgery involving the pelvis, abdomen, or reproductive organs
How long do sperm live inside the female body?

There is always a huge misperception about how long sperm can survive outside the man’s body and when they enter into the female reproductive tract. Many women are told all manner of untruths of sperm lasting for weeks at a time. The truth is that sperm cannot survive for long once they are released.

Precisely how long they can survive depends on the environment that they are released into and how quickly the fluid surrounding the sperm cells dries up.

Sperm lifespan inside the female body

After ejaculation, sperm may be able live inside the female body for several days, but that is dependent on many varying factors once they enter a woman’s body.  To be honest, most of the 300-500 million sperm are dead with an hour. That is a fact.

The fluid in a woman’s reproductive tract, especially the fallopian tubes, has all of the nutrients that sperm need for their survival during that time. But while the woman’s body can help sperm on their way to meet the egg, it can also hinder it their survival as well.

An egg has 24 hours to be fertilised, otherwise it dies

But even so, sperm really only have about 24 hours to fertilise an egg once it is released. After 24 hours, if the egg isn’t fertilised it will die, so really, it doesn’t matter how long the sperm can survive for if the egg has already died.

Poor sperm have to contend with many things when they enter a woman’s body. A woman’s vagina is coated in acids, to protect her from infections and bacteria, but it is also lethal to sperm. This is why within minutes and hours, most of the 300-500 million sperm that set off in search of the egg will be dead. Only a few million will survive to swim through cervix.

Climax helps with getting sperm into the uterus

A woman’s body can help to get the sperm going up into to the cervix though. Through climax (orgasm) contractions are created that can help pull sperm up and into the uterus. Through these contractions the cervix is dipped time and time again into a pool of waiting sperm and this then helps carry the sperm up into the second stage of their journey through the uterus and then up into the tubes.

Only a small number is sperm survive the journey

Once inside the female reproductive tract, the sperm cells must swim through the cervix and into the uterus to reach the fallopian tubes and then on to find the female egg. It is a very long journey for sperm to make and very few survive. Many get lost inside the uterus and some are attacked by the woman’s immune system along the way. By the time the survivors make it to the fallopian tubes to have a rest, there will only be less than 20 or more left to make the final journey.

After the sperm have a rest in the tubes and actually feed off some of the nutrients in the tubes many more will die, or be lost inside the tubes and by the time the final sperm reach the egg, there will be less than 10 single sperm left. Only one may then go on to fertilise the egg and an embryo is then started to be created.

All men should have there sperm quality tested

This is why all men need to have their sperm quality checked by a proper andrology lab and some men will need further testing of the DNA (DNA fragmentation analysis) and further genetic testing if the semen results are poor.

This should all be done prior to trying to conceive, as 50% of fertility issues are related to men. If a man has any of these risk factors, he should try to change them at least 3-4 months before trying to conceive, since that is how long it takes for sperm to fully mature. Some men may need longer than this, depending on what is causing their sperm to be of poor quality.

Incontinence and bladder issues

Incontinence, Bladder issues and Weak Pelvic Floor

Incontinence is often an embarrassing condition that will have more than a third of people suffering in silence because they are too embarrassed to seek treatment. Both men and women can suffer incontinence.

Although patients don’t die of incontinence, they often can’t live a fully productive life, they may have to curtail their workload or change jobs. Many factory workers and school teachers are only allowed set times for toilet breaks, so patient with urge incontinence may have to change jobs.

Patients who work in the military or the police force may have to resort to desk jobs. Too many women with incontinence stop having sexual intercourse, either because they are afraid they will leak during sex, or else they actually do leak, either at penetration, or at time of orgasm, which can be very disastrous. While men can also suffer incontinence, I am mainly going to focus on the causes of incontinence in women for this post.

Many people do not seek treatment

Many people don’t seek treatment because they also believe that surgery is their only option. Nothing could be further from the truth and surgery should only be used as a last resort once conservative methods have been used and aren’t working.

Even then, the surgical approaches used today are less invasive, are very effective and the recovery is very quick. It is not like it was 10-20 years ago. Keyhole surgery has really made major changes in this area and new surgical techniques are so highly effective.

I know people want to avoid any surgery, but sometimes it is needed and these days the recovery rates are so quick. A few days out of your life for recovery, can actually change your whole life. I just do not understand why anyone would put up with a life with incontinence, prolapse, or weak pelvic floor, when these issues are so easily fixed these days. I know many patients are so amazed at how easily their incontinence issues was fixed and how amazing they feel in getting their life back again. No more leakage when they laugh, cough, or exercise.

Incontinence does not have to be a normal part of life

Some people never seek treatment believing that incontinence is just a normal part of life. Again, this is not true and all and I encourage anyone with incontinence to talk to Urodynamic/Pelvic floor specialist, so they can help treat you and can also refer you to the right people for treatment and management if needed.

I know the pelvic floor/urodynamic surgeon I work closely with is amazing and what he can do for women and these issues is amazing too. Then I just help with the management on a more multimodality approach, moving forward.

Surgery may be necessary, but not always

Like I said before, surgery isn’t always needed. Many times conservative measures such as pelvic floor exercises, core strengthening, bladder toning, topical estrogen therapy, internal TENS (electrostimulation) and pulse magnetic therapy can all product fantastic long term treatment to this often debilitating condition, without the need for surgery.

There are also some special rings and other devices that can be used to prevent leakage and support prolapsed bladder and also vaginal prolapse causing incontinence. Men with incontinence also have options at hand and these can also be explored well before surgery is needed.

What people need to be aware of too, is that if you have been doing all the conservative treatments, and they aren’t helping, then it is time to get some surgical intervention. I think people think that pelvic floor exercises with fix all bladder and pelvic floor issues, and this doesn’t work, then there is nothing that can be done to help them. I need for all women (and men) to know that there is always help for bladder and pelvic floor issues and you just need to see a specialist, not just your GP.

Natural Medicines

On a natural medicine level, acupuncture and chinese herbal medicines may assist in the treatment and management of pain, alongside medical treatments and interventions.

Yoga and Pilates may also help toning of the pelvic floor and help managing incontinence, pelvic floor and post-surgical management of prolapse too.

How incontinence and other bladder issues is defined

Incontinence and bladder issues are defined as needing to pass urine more than 8 times per day, leakage of urine through cough, sneeze, urge, or without cause. It is important to seek help if you notice damp underwear, need to use pads because of leakage or are constantly running to the loo to pass urine.

The different types of incontinence

There are many different types of incontinence with the main ones defined as stress incontinence, urge incontinence and voiding dysfunction/incomplete emptying.

There is also mixed incontinence (mix of the 3 main ones) and also a term called overactive bladder syndrome which can be a mixture of all forms of incontinence. There are also inflammatory bladder conditions that cause incontinence such as bacterial cystitis and interstitial cystitis.

Physical issues such as previous surgery, childbirth and prolapse can also cause incontinence too.

The first task for the clinician is to find out how severe the incontinence is, based on the frequency of leakage, whether the woman finds it necessary to use incontinence pads, and if so how many pads. Some patients may prefer to change their underwear more frequently, while others may tuck tissue paper inside their underwear, and just throw away the tissues whenever they are damp.

The classic feature of stress incontinence is that the patient leaks with coughing, sneezing, laughing, running, playing sport or lifting heavy objects

The classic feature of urge incontinence is that the patient rushes to the toilet with an urgent desire to pass urine, but as she gets to the loo and is taking down her trousers, the urine comes away from her- sometimes before she has even sat down. Unfortunately these patients cannot predict when these bladder spasms will come upon them, and therefore can’t really tell when they are likely to leak.

Nocturia is defined as being woken up by your bladder needing to go to the toilet- as opposed to being woken up by a crying baby, a snoring husband, or menopausal night sweats. However nocturia is age dependent. Nocturia is defined as waking 1 or more times per night if under 60 years of age

Typically a patient with Voiding dysfunction/ Emptying difficulty has to strain to commence voiding (called “hesitancy”). They may also have observed that when they compares themselves to other women urinating in the toilets at the movies or in airports, her stream seems poorer than others with the urine dribbling out more slowly. These women will often also describe the sensation of incomplete emptying and may need to go back to the toilet within a relatively short time to re-void. Sometimes these women will leak as they get off the toilet, which is how the realise that they are not empty. This is called post-micturition dribble incontinence.

Incontinence in males

In males these symptoms most commonly occur when the prostate gland is enlarged- causing a relative urethral obstruction and making it more difficult for the urine to get out. These men have chronic high residual urine, so they end up going to the toilet very frequently in a vain attempt to empty out. They often dribble urine onto their clothes. When such men have prostatic surgery they usually find that their urine flow rate returns to normal and they usually stop dribbling.

Inflammation in the bladder leading to pain

We will also need to explore how inflammation of the urinary bladder leads to suprapubic pain, and consider the two main causes of this, Bacterial Cystitis and Interstitial Cystitis .It often feels difficult to treat incontinence in the presence of bacterial cystitis, because such patients are overwhelmed by frequency and urgency of micturition and they may experience disabling suprapubic pain. It is fixable with the right treatment though. It all gets back to who is managing you.

On the other hand, a separate cause of Urinary Pain, called Interstitial Cystitis, does not usually cause much incontinence at all- it just causes suprapubic (bladder) pain with severe frequency and urgency of micturition. You can read my post about Interstitial Cystitis

The overactive bladder (OAB) is a clinical syndrome, not a urodynamic diagnosis. It comprises frequency, urgency, and nocturia, with or without urge incontinence.

Gynaecological conditions such as Endometriosis and Adenomyosis etc can also cause issues with bladder and pelvic floor.

Solutions for incontinence, bladder issues, & pelvic floor issues

No matter what sort of incontinence you have, there is always an answer and a solution to your problem. Not all solutions are surgical either. More often than not some conservative treatments, some exercises and some complementary therapies is all that is needed.

Sometimes all some women need is some treatments with estrogen creams to help with tone in the vagina, bladder and pelvic floor

Surgery and now bionic devices are always an option for those whom have tried conservative options and aren’t getting the desired results. Surgery is often used because of quality of life issues. Again surgery these days is so effective and less invasive and the recovery is so much quicker due to keyhole surgery and new surgical interventions.

Many times there may be an easy non-invasive treatment for your particular issues. Even when surgical intervention is needed, these days even this is less invasive than it used to be and the recovery and results are very quick.
